And more importantly, what health care decisions would you want them to make?”

Advance care planning is invaluable because it ensures your voice is heard if medical decisions have to be made for you. Having a plan means your friends, family, doctors and health team aren’t guessing about what’s important to you or what an acceptable outcome is for your life. The plan can then be uploaded to your My Health Record and your Queensland Health Record if it is needed for your health care.
